29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:     A.)  Lynaugh Brothers Construction, Inc., 8205 Peach Street, Erie, PA  16509, fuel pump canopy area, on or about June 5, 2013 and times prior thereto: Head protection was not utilized by company personnel when they were potentially exposed to falling objects from construction work operations taking place in elevated locations.  These locations included but were not limited work taking place from aerial lifts and from the top of the canopy.

Lynaugh Brothers Construction, Inc., 8205 Peach Street, Erie, PA  16509, fuel pump canopy area, on or about June 5, 2013 and times prior thereto:  Employee working in the basket of a JLG 60H aerial lift were wearing fall protection harnesses, but were not tied into the basket via a lanyard.
