Lynaugh Brothers Construction, Inc., 8205 Peach Street, Erie, PA 16509, fuel pump canopy area, on or about June 5, 2013 and times prior thereto: Head protection was not utilized by company personnel when they were potentially exposed to falling objects from construction work operations taking place in elevated locations. These locations included but were not limited work taking place from aerial lifts and from the top of the canopy.
