29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights was no more than 1 inch:     (a)  On or about May 24, 2013, and at times prior, Mountain States Construction Company, Inc., as the controlling employer, did not ensure that employees of Tracy Travis, subcontractor, were protected from fall hazards from a rolling scaffold that was not fully planked on the working levels. 29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a)  On or about May 24, 2013, Mountain States Construction Company, Inc., as controlling employer, did not ensure that employees of Tracy Travis, subcontractor, were protected from falls of approximately 18 feet through the roof access hole that was not guarded while they were working on the roof. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  On or about May 24, 2013, Mountain States Construction Company, Inc., as controlling employer, did not ensure that employees of Tracy Travis, subcontractor, were protected from fall hazards of approximately 18 feet while working at the edge of the roof of a building under construction without fall protection.      Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    Mountain States Construction Company, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 911911547, citation  number 1, item number 2a and was affirmed as a final order on July 28, 2009, with respect to a workplace located at 2310 W. 9th Avenue in Longmont, Colorado.
