Broadway, Wichita, KS 67211 - Employees were exposed to fall hazards of 12 feet to the ground below. a) Cross braces of the tubular welded frame scaffold were used as a means to gain access onto and off of the scaffold. b) End frames of the scaffold were used as a means to gain access onto and off of the scaffold. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
