29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:  QT #326, 740 S. Broadway, Wichita, KS 67211 - Employees were exposed to fall hazards of 12 feet to the ground below.  a)  Cross braces of the tubular welded frame scaffold were used as a means to gain access onto and off of the scaffold.  b)  End frames of the scaffold were used as a means to gain access onto and off of the scaffold.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  QT #326, 740 S. Broadway, Wichita, KS 67211 - Employees were exposed to fall hazards of 12 feet to the ground below while moving stones, planks, and materials on the top level of the tubular welded frame scaffold.  The employees were not working under the supervision and direction of a competent person at the job site to recognize hazards and procedures to control or minimize those hazards.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

Broadway, Wichita, KS 67211 - Employees were exposed to fall hazards of 12 feet to the ground below.  South side of the building - Guardrails were not provided to protect employees when they were walking back and forth on the top level of the tubular welded frame scaffold removing stones.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
