29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. On or about June 6, 2013; at the center of the jobsite, the employer allowed employees to utilize an extension cord that was missing a ground pin. The extension cord was used to deliver power from a temporary power distribution panel to a portable radio. The missing ground pin prevented the extension cord from having a continuous path to ground.
