29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) An employee was unloading materials from a Sky Track #6036 lift while on a steep roof and was not protected from falling by a personal fall arrest system or other form of fall protection. The worker was exposed to a 20 foot fall hazard.

29 CFR 1926.1053(b)(1): 29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigged support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) A ladder was used that was not long enough to extend three feet above the eave of the roof which was the upper level landing surface and the ladder was not secured at its top and a grasping device was not provided to assist employees in mounting and dismounting the ladder.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: (a) An extension ladder used to gain access to a roof was set at an angle less than one-quarter of the working length of the ladder. The working length was 20 feet and the ladder base distance out from the support point was approximately 3.5 feet.
