29 CFR 1910.29(b)(2): Scaffold was not properly braced by cross braces and/or diagonal braces for securing vertical members together laterally. The cross braces shall be of a length that will automatically square and align vertical members so the erected scaffold is always plumb, square, and rigid. a) An employee used a 7 foor high, metal, mobile scaffold where the diagonal braces on the one side were missing, thereby exposing the employee to a hazard, observed 5-29-13. CSHO observed abatement.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment. a) Ceiling Set Area - An electrical panel box, PPL1-2B, labeled as 120v-208v 3 phase, did not have an unused circuit breaker opening covered, thereby exposing employees to the hazards associated with coming in contact with energized parts, observed 5-29-13. CSHO observed abatement.
