OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards created by the use of industrial storage racks which were not anchored to the floor:    a) North Building, 97 Pines Marginal Road, Charleroi, Pennsylvania 15022:  Observed on or about May 30, 2013, industrial storage racks used to store various types of well head equipment were not anchored to the floor.  The industrial storage racks were used to store low crosses and crates of studs and nuts.  Employees using a forklift to load these materials onto or remove these materials from the industrial storage racks were exposed to crushed-by and struck-by hazards.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer fails to make necessary repairs, resulting in the operation of a crane with defective or missing parts affecting the safe operation of the crane and likely to result in serious injury or death to employees in the event of crane malfunction or failure:    a) Main Shop, 97 Pines Marginal Road, Charleroi, Pennsylvania 15022:  Observed on or about May 30, 2013, maintenance and repairs had not been conducted for the Contrx jib crane (Model Number A41212, Serial Number 45823), resulting in operation of the crane with a serious defect:    1)  The latch on the hook was missing.

29 CFR 1910.178(a)(4):  Modifications or additions which affect the capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval:   a) Main shop, 97 Pines Marginal Road, Charleroi, Pennsylvania 15022:  Observed on or about May 30, 2013, a 1 and 1/4 inch hole was drilled into each fork of a Toyota forklift (model number 7FDKU40).  The modification was not approved by the manufacturer.  Employees using the forklift to lift well head components were exposed to struck-by hazards.
