Giddens Ave., Tampa, FL 33610, as observed on or about 06/06/2013, employees were exposed to trip/fall hazards due to 4 ft. high stairs containing 5 risers that were missing a railing on both of their sides.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire; or in an acetylene generator compartment. a) For the Accurate Paper Recycling facility at 5500 E. Giddens Ave., Tampa, FL 33610, as observed on or about 06/06/2013, two oxygen compressed gas cylinders were chained together with an acetylene fuel gas cylinder in the maintenance shop.
