29 CFR 1926.102(a)(1): Eye and face protective equipment was not provided when machines or operations presented potential eye or face injury: (a) Bost & Young LLC dba Rick Bost Roofing at 406 13th St, Glenwood Springs, CO 81601: On and before June 4, 2013 the employer did not ensure employees were provided eye protection while operating pneumatic nail guns while installing roof shingles. Both employees were operating pneumatic nail guns and were not wearing eye protection. This condition exposed employees to projectile hazards to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Bost & Young LLC dba Rick Bost Roofing at 406 13th St, Glenwood Springs, CO 81601: On and before June 4, 2013 the employer did not ensure that employees were protected from injury, in that both employees installing shingles on the second level roof on a two-story residential home were not utilizing a fall protection system. The pitch of the roof was 6-in-12. This condition exposed employees to a fall hazard of approximately 18 feet.
