29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: (a) On or about 6/6/2013, on the west side of the building, at least one employee performing framing work while standing on a job-made work basket that had not been designed by a qualified person was exposed to the hazard of falling to the lower level.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a) On or about 6/6/2013, on the west side of the building located at 4700 S. Cooper, Arlington, Texas, an employee was working from a wood box elevated by a 10054 JLG aerial forklift. The wood box was not an approved work platform by the manufacturer for capacity, construction, use, and fall protection.
