29 CFR 1926.1433(d)(4)(iii): The crane hook safety latches did not adequately close the throat opening and be designed to retain slings or other lifting devices/accessories in the hook when the rigging apparatus is slack. At the jobsite; employees used a Hylab 218, HSL Series 110 ton crane with defective crane hooks that did not adequately close during rigging and lifting 11-12 feet long steel rebar that weight approximately 4,032 pounds, exposing employees to struck-by hazards.
