29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees: a) On or about June 06, 2013, at 2902 Dabney Dr. Pasadena, TX; the insulation was removed from a portion of the flexible cord of a circular saw. This condition exposed the employees to electric shocks.
