29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On June 5, 2013, at the job site on the West side of a wood frame building, untrained employees were working at heights greater than 30 feet without fall protection, exposing employees to fall from heights.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. At the job site, on June 5, 2013, employees did not have adequate fall protection while working from a roof with a slope 4:12, at an approximate height of 30 feet, exposing employees to fall hazards. The Roger Ibarra was previously cited for a violation of this occupational safety and health standard 1926.501(b)(10), which was contained in OSHA inspection number 313038572, citation number 1, item number 1, and was affirmed as a final order on June 29, 2009, with respect to a workplace located at 15903 Yorktown Crossing Parkway, Houston, Texas 77084.
