A stairway, ladder, ramp or other safe means of egress were not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a) 52 North Broadway, White Plains, NY exterior of catacomb of church of the Sisters of the Divine Compassion. There was only one ladder in an excavation that was 79 ft. long which required employees to travel more than 25 feet to egress.

Observed on or about 05 June 2013.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a) 52 North Broadway, White Plains, NY. Employee was working inside of an excavation with a spoil pile less than 2 feet from the opening without a hardhat. Observed on or about 05 June 2013. J. Salvatore & Sons, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 311284988, citation number 1, item number 01 and was affirmed as a final order on 6/20/2009, with respect to a workplace located at 58 Farragut Avenue, Yonkers, New York 10706.
