29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean. a) Wood Shop: The employer failed to ensure that the floor, piping, walls and equipment remained free from the accumulation of combustible dust (wood). Employees were exposed to a fire/explosion hazard while performing duties involving the cutting of wood doors and window pieces to make custom designs. A sample taken on June 06, 2013 was determined to be an explosive CLASS II wood dust.

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location Wood Shop: The employer failed to ensure that all electrical equipment operated in the area was approved for a DIVISION 1,CLASS II location. Employees were exposed to a fire/explosion hazard while cutting wood material to custom design doors and windows. A sample taken on June 06, 2013 was determined to be a CLASS II explosive material. a) Florescent lighting was utilized for overhead lighting in the area. b) Lasko fan: Model # NSC-243 was not explosion proof. c) Shop Vac vacuum: Model # QL650-1C was utilized to remove dust from the floor.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area: a) Wood Shop: The employer failed to ensure that all exposed employees were provided training on the hazards of combustible dust. Employees were exposed to a fire/explosion hazard while performing duties involving the cutting of pieces to custom design windows and doors. A sample taken on June 06, 2013 was determined to be CLASS II combustible dust. ABATEMENT CERTIFICATION AND DOCUMENTATION NOT REQUIRED
