On or about June 06, 2013.    b) Third floor -  The Simplex Rubber Cutter (model RB-4) was not equipped with a guard to protect employees from point of operation and rotating part hazards, while cutting rubber.  On or about June 06, 2013.    c) Third floor - The unguarded Craftsman 150 drill press chuck exposed an employee to a rotating part hazard, while drilling holes.  On or about June 06, 2013.
