1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:     a.  On or about June 6, 2012, at the above addressed jobsite, employees installing netting from a JLG aerial lift, approximately twenty (20) feet above the ground, were not wearing full body harnesses, thereby exposing the employees to falls.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
