29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about June 5, 2013, and at times prior thereto, employees engaged in mixing into and applying Klean-Wall with a pump sprayer were not provided and required to use chemical splash goggles.  Klean-Wall is corrosive containting sulfuric and phosphoric acids.  Abatement certification is required for this item.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  a) On or about June 5, 2013, and at times prior thereto, there was no guardrail or other means of fall protection for the storage mezzanine located above the office and restroom.  Workers using an extension ladder to access the area for equipment and tanks were exposed to an 8.5 to 9 feet fall hazard to the concrete surface below.  Abatement certification is required.
