29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system. (a)Rivera Concrete Foundation @ 3988 Mt. Hope St., Westminster, CO 80549: On June 4th, 2013, the exposing employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system, in that, employees were engaged in setting concrete formwork operations while standing on top of the formwork exposed to a 10 foot fall hazard. This condition exposed the employee to a fall hazard of 10 feet.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: (a) Riveria Concrete Foundation @ 3988 Mt. Hope St., Wellington, CO 80549: On and before June 5th, 2013, the exposing employer did not ensure subcontractor employees were protected from impalement hazards with exposed vertical rebar within a residential excavation while walking and working on the surface excavation. Employees walked and worked near the edge of the excavation. The excavation was approximately 7 feet in depth. This condition exposed employees to impalement hazards.
