29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:       a. On or about June 6, 2013, at the above addressed jobsite, employees were installing a sewer in an excavation measured at six (6) feet in depth.  The excavation was not provided with cave in protection, thereby exposing the employees to cave in hazards.      The Stenstrom Excavating & Blacktop, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 314037565, citation number 1, item number 3a and was affirmed as a final order on June 13, 2012, with respect to a workplace located at 1006 Auburn St. Rockford, Illinois.
