29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. a. On or about June 5, 2013 - at the above addressed jobsite, an employee working in a JLG E450AJ telescopic boom aerial lift was not attached to the boom or basket by the use of a lanyard, thereby exposing the employee to the hazards associated with a fall of approximately forty eight (48) feet.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: a. On or about June 18, 2012 - at the above addressed jobsite, an employee using the JLG E450AJ Telescopic Boom Aerial Lift was not trained to recognize the hazards associated with the use of aerial lifts, thereby exposing the employee to the hazards associated with falls.
