29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet): a) Bldg 88A - Employees were performing warehouse work activities in an area that had three unmarked doors that did not have "Not an Exit" signage, on or about May 24, 2013. Employees stated that in the event of an emergency, they would utilize the unmarked doors to evacuate the facility. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Bulk Section Area F - Employees were conducting work activities in an unmarked area that had a large acid stain as a result of a battery charger malfunctioning and their employer did not provide an eyewash station on or about May 24, 2013. b) Upper Warehouse, Battery Charging Area -Employees were hooking up batteries that had acid seeping out at its edges to battery chargers and their employer did not provide an eyewash station in the area on or about May 24, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1960.8(a): 29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to a biological hazard: a) On or about May 24, 2013, and at times prior, Bulk Section, warehouse staff are exposed to a variety of illnesses, including but not limited to ornithosis, encephalitis, newcastle disease, toxoplasmosis, salmonella, and histoplasmosis when the employer did not ensure that the warehouse and its contents were adequately maintained in a clean and sanitary condition free from a build-up of bird feces. The warehouse staff was being made to clean up large amounts of bird feces from their work floors, desk, chairs and handle barrels, crates and supplies contaminated with bird feces. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
