29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.     Worksite:  The employees, working on the low pitched roof were not protected from the hazard of falling approximately 14 feet 7 inches to the ground by the use of appropriate fall protection devices and/or systems.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   Worksite:  The employer did not document in writing that the employees had been adequately trained in the hazards associated with falling and the proper ways and procedures to take in order to minimize these hazards.
