While the cover is not in place, the floor hole was not constantly attended by someone or was not protected by a removable standard railing:      The employer does not guard floor holes. This violation was observed on June 10, 2013, at a workplace located at 2856 Fort Worth Avenue, Dallas, Texas; where employees were exposed to trip and fall hazards from unguarded drains in the processing area.

This violation was observed on June 7, 2013, at a workplace located at 2856 Fort Worth Avenue, Dallas, Texas 75211; where employees ascended and descended a stairway that was installed at a 55 degree angle to the horizontal.  The employees were exposed to the hazard of falling to the lower level.
