Location: BYK USA, 524 South Cherry Street, Wallingford, CT 06492    a) The 7 foot 2 inch wide, 7 foot 5 inch long, six frame high (6 foot 7 inch high per frame) tubular welded frame scaffold in the freight elevator shaft was not equipped with a fall protection system while employees were erecting and dismantling scaffold components.    b) The 7 foot 1 inch wide, 7 foot 2 inch long, six frame high (6 foot 7 inch high per frame) tubular welded frame scaffold in the pallet elevator shaft was not equipped with a fall protection system while employees were erecting and dismantling scaffold components.    Consequently, On or About June 5, 20013, a laborer erecting and dismantling scaffold components 21 feet 10 inches above the concrete floor of the freight elevator fell to the concrete floor sustaining serious bodily injuries.    B.W. Dexter II Incorporated was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vi), which was contained in OSHA inspection number 311782353, citation number 01, item number 003 and was affirmed as a final order on June 20, 2010, with respect to a workplace located at 226 Putnam Pike, Dayville, CT 06241.
