Condition exposed employees to entrapment and collapse/cave ins while working in a trench greater than 5 feet in depth.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. (a) Van Matre Construction, LLC, 1900 Plaza Drive, Louisville, CO 80027; On and before June 10, 2013, the exposing employer failed to ensure that excavated or other materials were at least 2 feet from the edge of the excavation or that some other protective system such as retaining devices were used. Condition exposed employees to struck by hazards.
