29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Facility, 150 Kriess Road, Butler, PA:  On or about 06/07/13 there was no adjustable tongue guard on the Delta Shop Master Bench grinder.  The grinder is used by employees to smooth ball allen wrenches and sharpen T-handles and there is no guard to prevent the tools from being caught in the wheel. The opening measured approximately 3/8" between the wheel and equipment frame.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a)  Facility, 150 Kriess Road, Butler, PA, Press P2:  On or about 06/07/13 no guarding was provided on the numbering unit, marginal power unit and #4 print tower exposing employees to the rotating rollers.   (b) Facility, 150 Kriess Road, Butler, PA, Press P7:  On or about 06/07/13 no guarding was provided on the marginal unit and printer towers exposing employees to the rotating rollers.   The employer, Wise Business Forms, Inc. was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 313790982, citation number 1, item number 3 which were issued on 11/19/09, with respect to a workplace located at 2301 Congress Street, Portland, ME.

29 CFR 1910.1200(h)(2)(iii): The employer did not provide information to the employees as to the location and availability of the written hazard communication program, and material safety data sheets required by 29 CFR 1910.1200:   (a) Facility, 150 Kriess Road, Butler, PA:  On or about 06/07/13 the employer did not inform the employees of the location and availability of the material safety data sheets.  Employees who use products such as inks, plate cleaner and roller cleaners did not know the location of where the material safety data sheets were kept.
