29 CFR 1910.147(c)(4)(i):     Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.       a) Worksite, Warehouse A/Polywrap Dept: Employees were required to provide repairs and maintenance on BUHRS 2000 Polywrap machines such as, but not limited to Machine #4 with numerous sources of energy such as electrical, pneumatic, and mechanical. The employer did not develop, document and utilize Lockout/Tagout procedures to control hazardous energy; on or about 3/27/18.    Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(IDENTIFY MACHINERY OR EQUIPMENT)    a) Worksite, Warehouse A/Polywrap Dept: Employees were required to provide repairs and maintenance on BUHRS 2000 Polywrap machines such as, but not limited to machine  #4 with numerous sources of energy such as electrical, pneumatic, and mechanical. The employer did not provide the employees with Lockout/Tagout training; on or about 3/27/18.
