29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) Elevator shaft foundation excavation area, 524 North Homewood Avenue, Pittsburgh, Pennsylvania 15219: Observed on or about June 10, 2013, the employer did not perform daily inspections of the excavation and did not perform soil classification tests of the soil. An employee engaged in form work near the bottom of a 6.5 deep excavation was not protected from cave-ins. The employee was exposed to crushed-by hazards.
