OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing and struck-by hazards associated with improper automotive lift operations:     (a)  On or about 10 June 2013, and at times prior to that date, employees were exposed to struck-by hazards while working around automotive lifts, in that the lifts were not being inspected periodically to ensure they were properly maintained and functioning properly.  Rotary 2-post lift in the service bay, Model # SP0A10N500, Serial # BVP04E0187 did not have the locks adjusted properly, creating an out of balance condition with the lifted vehicle.     (b)  On or about 10 June 2013, and at times prior to that date, employees were exposed to struck-by hazards while working around automotive lifts, inn that they were not trained in the proper use and safety features of automotive lifts.  An employee was working under a raised vehicle that was not resting on the safety locks and when asked to lower the vehicle it was discovered one lock was not engaged.       Abatement note:       Among other methods, one feasible and acceptable method of abatement would be to follow the requirements of ANSI/ALI and/or ANSI B153.1 which includes, among other requirements, the following:        1.  Institute a preventive maintenance program according to the manufactures' specification.        2.  Schedule and complete all periodic and annual inspections.       3.  Implement and document an employee training program.
