29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a. On or about June 10, 2013, an employee working below employees on the roof was not wearing a hard hat, exposing himself to the hazard of being struck in the head by falling objects.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision of paragraph (b) of this section:     a. On or about June 10, 2013, on a 5:12 pitch roof, six employees performing roofing work were not protected from a 7-foot, 10-inch fall hazard by the use of a fall protection system.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed    a. On or about June 10, 2013, a ladder extending from ground level to the roof peak was not set with the side rails extended at least 3 feet (.9 m) above the upper surface, exposing employees to an 11-foot, 8-inch fall hazard.
