29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe. At the job site, on June 7, 2013, an employee was using a Makita Circular Saw model 5007F with the blade guard shimmed back, exposing the employee to amputation hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcture, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On June 7, 2013, at the job site, in the front of a wood frame home employees did not have a ladder to access the job built scaffold, exposing employees to falls greater than 16 feet.
