29 CFR 1910.178(a)(5): Powered industrial truck(s) equipped with front-end attachments other than factory installed attachments require the user to request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) Warehouse & Showroom Areas- The employer failed to ensure that powered industrial truck(s), such as the Hyster Forklift 1S, that were equipped with front end attachments other than the factory installed attachments, such as the clamp attachment which was used to unload containers, were marked to identify the attachments and show the approximate weight of the truck and the attachment combination at the maximum elevation with the load laterally centered. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Such examination shall be made at least daily: a) Warehouse & Showroom Areas- The employer failed to ensure all forklifts, such as but not limited to the Hyster & Nissan Forklifts, were examined prior being placed into service and if the examination shows any condition that could adversely affect the safety of the vehicle, such as non-functioning head lights and brake lights, that the truck(s) not be used. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
