29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) At the worksite, on April 24, 2013, an employee was preparing the exterior of the builiding for siding installation from the pump jack scaffold approximatly 14 feet from ground. The scaffold was missing a midrail and enrails along the sides and the ends, exposing the employee to falls.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary: a) At the worksite on April 24, 2013, the top portion of the scaffold was attached inadequately at the top and the bottom was not secured to the structure. This condition exposed the employee to sccaffold collapse.

29 CFR 1926.501(b)(14): The employer did not ensure that each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) At the worksite, on April 24, 2013, an employee was installing siding to the second level patio, approximately 12 feet above ground level. The employee was not protected against falls.
