29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors: a) For the Free Press Publishing Company facility at 1010 West Cass St., Tampa, FL 33606, as observed on or about 06/11/2013, employees were exposed to fire hazards associated with the use of an electric drill to mix flammable ink/paint with flammable solvents such as mineral spirits. A sparking ignition source was present from the drill's electric motor to ignite flammable vapors present.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4): For the Free Press Publishing Company facility at 1010 West Cass St., Tampa, FL 33606, as observed on or about 06/11/2013, no lockout tagout program was present and being used for employee maintenance activities. Employees performed maintenance tasks such as, but not limited to, changing belts, chains, and lubrication on the following equipment. a) Folder - electricity, 240/480 VAC, compressed air, moving parts/mechanical energy b) Stitcher - 240/480 VAC, moving parts/mechanical energy c) Press/non-heated - 240/480 VAC, moving parts/mechanical energy d) Press/heated - 240/480 VAC, moving parts/mechanical energy, thermal energy
