29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about June 11, 2013, employees were exposed to struck by and crushing hazards while using an Allis-Chalmers forklift, model ACP70 L PS Dula, serial number AAH113296 in that additions and modifications which affected the capacity and safe operation of the forklift were made. The manufacturer's label plate listed 54 inch forks as attachments. However, the forklift was equipped with 74 inch forks and a trailer hitch attachment that was used to move Travelift trailer, model T300, serial number T1051280. Additionally, the forks had been modified with holes to retain the trailer hitch attachment.
