29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a. Examples existed of equipment being used with missing equipment grounding conductor pins ("ground pins"), such as, but not limited to, the DeWalt abrasive wheel cut-off saw, Bosch portable plunge router, and Delta band saw. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity: a. In the saw department, a 110 V electrical outlet, used to power equipment such as a router and band saw, was wired with reverse polarity. Abatement certification and abatement documentation is required for this violation.
