29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:   a) Job Site, Building C:  Plywood, planks, foam insulation and cinder blocks were scattered under the condo being framed by three employees, on or about 6/6/13.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a) Job Site, West area, Building C:  Three (3) employees framing a condominium ranging from 25' to 30' high above the ground, were not protected from falling, on or about 6/6/13.
