OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of crushing or death by heavy equipment: FACILITY: The employer did not ensure the rear end of the Bandit 200 Brush Bandit wood chipper, Company unit number 660290 was appropriately supported and/or braced while the employee was working underneath to perform a "brake job". On or about June 11, 2013, the employee was working underneath the wood chipper sustained serious injuries when the unsupported rear end of the wood chipper tipped over.
