29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about June 12, 2013, at the south side of the building, one employee installing metal lathe on an exterior wall from a 7:12 pitch roof was not protected from a 10 feet fall hazard by a fall protection system.      b.  On or about June 12, 2013, at the west side of the building, one employee installing metal lathe on an exterior wall from a 7:12 pitch roof was not protected from a 10 feet fall hazard by a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about June 12, 2013, at the west side of the building, one employee doing lathe work on the exterior wall did not set up the Werner 24-ft. aluminum extension ladder at least three (3) feet above the landing surface, exposing himself to a 10 feet fall hazard.

On or about June 12, 2013, at the southwest side of the building, an employee installing lathe on an exterior wall was using a closed Werner stepladder against the wall, exposing himself to a fall hazard.
