29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm): On the jobsite located at 4983 Chippewa, Saint Louis, MO 63109, an employee was observed engaged in tuckpointing on a platform of a tube and coupler scaffold at greater than 25 feet from the ground that was not fully planked.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On the jobsite located at 4983 Chippewa, Saint Louis, MO 63109, the employer did not provide fall protection for employees working more than 10 feet above lower level. An employee was observed engaged in tuckpointing on a platform of a tube and coupler scaffold greater than 25 feet from the ground with the use of fall protection.
