29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     The employer failed to protect employees from potential amputation hazards associated with operating a piece of equipment with rotating parts and no guarding.   This was most recently evident during the inspection of the tin can packaging line in the facility located at 16-701 Macadamia Rd., Keaau, HI 96749, where the employer has employees operating lathes without guards.  Instances include but are not limited to:     i.    Employees operating the Walcott Lathe (no data plate = no serial number or model number) without a guard over the rotating chuck as they fabricate replacement parts for equipment throughout the facility.   ii.   Employees operating the AndyChow Lathe model number  TUG 40/80, serial number  07/85 5820 without a guard over the rotating chuck as they fabricate replacement parts for equipment throughout the facility .

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:    The employer failed to protect employees from potential amputation hazards associated with using inadequate guarding of belt and pulley assembly.  This was most recently evident during the inspection of the tin can packaging line in the facility located at 16-701 Macadamia Rd., Keaau, HI 96749, where the employer has employees lifting up the unsecured access door to the vertical conveyor system to remove tin cans that have fallen down and pose a jamming potential near the bottom belt and pulley assembly.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:     The employer failed to protect employees from potential electrical hazards associated with using electrical components in a manner for which it was not designed to be used by the manufacturer.  This was most recently evident during the inspection of the facility located at 16-701 Macadamia Rd., Keaau, HI 96749, where the employer has employees using office automation equipment and other electrical components plugged into a portable power tap (power strip) that is plugged into another portable power tap in a practice commonly referred to as daisy chaining; in this instance the first PPT that has all of its plugs full along with the plug from the second PPT and its three components plugged in, creating the potential for overloading the outlet and injuring an employee.
