Instances include but are not limited to:     i.    Employees operating the Walcott Lathe (no data plate = no serial number or model number) without a guard over the rotating chuck as they fabricate replacement parts for equipment throughout the facility.   ii.   Employees operating the AndyChow Lathe model number  TUG 40/80, serial number  07/85 5820 without a guard over the rotating chuck as they fabricate replacement parts for equipment throughout the facility .
