29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   An employee using a JLG aerial lift to gain access to the roof on the south side of a residential building were exposed to a fall hazard of approximately 20 feet due to the lanyard on the safety harness not being tied off to the boom or basket.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: An employee inside a JLG aerial lift basket was not tied off to anything when he stepped out of the JLG aerial lift basket and onto the south side of a residential building roof at a job site located at 813 N. 9th, Kansas City, KS.  The employee was exposed to a fall hazard of approximately 20 feet to the the ground below.
