29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) On or about June 12, 2013, at the above addressed jobsite, an employee working from a scaffold plank set on the top frame of a tubular welded frame scaffold was exposed to a fall of 22 feet to the ground from the ends of the scaffold. b) On or about June 12, 2013, at the above addressed jobsite, an employee was working from a tubular welded frame scaffold which had no guardrails on the ends and a cross brace along the back which acted as a toprail. The employee was exposed to a fall 15 feet to the ground. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) On or about June 12, 2013, at the above addressed jobsite, employees climbed tubular welded scaffold frames to access their work platforms and were exposed to a fall of 6 feet to the canopy on which the scaffold was placed. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
