29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of job sites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying exit:     a) M.L. Ruberton Hillside, NJ site -  Route 22 East bridge:     Employer failed to implement and enforce the company's Health and Safety Policy with regard to Fall Protection at the work site. Frequent and regular inspections of the site were not conducted to ensure that employees work was performed in a safe manner.      Condition noted on or about 06/12/2013.

Ruberton Hillside, NJ site - Route 22 East bridge:       Employees performed work on the outer edge of a bridge wall estimated to be approximately  50 - 60 ft. above the asphalt road surface below without proper personal fall arrest systems.
