29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) North-east wall, machine dept, Niles unmarked horizontal lathe: On or about June 12, 2013 employees operating the Niles lathe were exposed to struck-by hazards where a chip guard was missing.  b) North-east, repair bays, state testing station, Wellsaw horizontal bandsaw model 1000: On or about June 12, 2013 employees cutting steel pieces and beams were exposed to amputation hazards where the unused portion of the blade was not guarded.  c) North-east, repair bays, state testing station, Wellsaw horizontal bandsaw model 1000: On or about June 12, 2013 employees were exposed to caught-on hazards where the idle and drive wheel guards were missing exposing two rotating flywheels.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder (handheld angle grinder) were not provided with safety guard having a maximum exposure angle of 180 degrees and located between the operator and wheel:  Welding dept. and repair bays: On or about June 12, 2013, employees using multiple portable handheld angle grinders (Makitas, DeWalt, Milwaukee) were exposed to laceration and struck-by hazards where adequate abrasive wheel guards were not provided.  There is no abatement certification or documentation required for this item.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire;   Outside tank storage trailer box: On or about June 12, 2013, 11 tanks (275 gallons) of Oxygen and 5 tanks (155 gallons) of Acetylene were stored together with other chemicals and combustible materials.  Abatement certification is required of this item in accordance with the requirements of 1903.19(c).
