29 CFR 1926.403(e): Splices. Conductors shall be spliced or joined with splicing devices designed for the use or by brazing, welding, or soldering with a fusible metal or alloy. Soldered splices shall first be so spliced or joined as to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors shall be covered with an insulation equivalent to that of the conductors or with an insulating device designed for the purpose. a) 4300 Veterans Blvd., Metairie, LA; On or about 6/12/13, the employer allowed employees to use an air compressor with the power cord spliced together with an extension cord. The splice was wrapped with plastic wire nuts and electrical tape that did not provide an adequate barrier to moisture.

29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units. a) 4300 Veterans Blvd., Metairie, LA; On or before 6/12/13, the employer allowed an employee to work from a scaffold that was on an unstable surface. The surface was uneven dirt with objects such as rocks, broken concrete, and other debris.
