29 CFR 1915.507(b)(1): Portable fire extinguishers and hose systems were not selected, installed, inspected, maintained, and tested by the employer according to NFPA 10-2002 Standard for Portable Fire Extinguishers: (a) Throughout facility; portable fire extinguishers, in locations such as but not limited to the shop on the office barge and the break room located in the hold of the crane barge Leonard, were not inspected in between annual maintenance provided by a third party. NFPA 10-2002, 6.2.1 requires fire extinguishers be inspected at approximately 30-day intervals, and 6.2.2. lists the minimum items to be checked during each inspection.

29 CFR 1904.40(a): The employer did not provide an authorized government representative copies of requested records kept under Part 1904 within four (4) business hours: (a) On or about June 10, 2013, the employer failed to provide copies of the injury and illness records to an authorized representative. The requested OSHA 300 and 300A forms, for calendar year 2010 to date, were provided by email on June 14, 2013, after the completion of the inspection, due to a several months long software problem.
