Yes, your local OSHA Area Office director will decide whether to investigate the incident, depending on the circumstances of the heart attack.  a. On or about May 6, 2013 an employee died of an apparrent heart attack, and the employer did not report the fatality to the Columbus Area Office or the toll free OSHA reporting line.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  a. At the site located at 4000 South High Street in Columbus, Ohio the employer had not completed a written certification of the hazard assessment for employees that would refinish wood floors.
