29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be me:    On or about June 14, 2013, the employer did not develop and implement a hazard communication program including a chemical list, making material safety data sheets available to employees, a hazardous chemicals training program, and ensuring that all chemicals were labeled for identity.  Chemicals at the facility included, but was not limited to, Acrylite acrylic sheets, Makrolon Virak thermoplatic polymer sheets, Weld On 3 adhesive for acylics, propane, and polycarbonate tinted sheets.    *Abatement certification is required per 29 CFR 1903.19(c).

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets:  On or about June 14, 2013, the employer did not provide employees training on the hazardous chemicals in the workplace, including, but not limited to, the Methylene Chloride in Weld-On 3 for Acrylics, propane, Acrylite acrylic sheets,, Makrolon Virak thermoplastic polymer sheets, and polycarbonate tinted sheets.  * Abatement certification is required per 29 CFR 1903.19(c).
